Background:

The Seminole County ARPA Community Health Initiative includes funding to contract community agencies to perform the Community Health Case Managers (CHCMs) duties.  Seminole County Community Services advertised a Notice of Funding Availability for these services and the responses did not meet the needs of the program.  The request is to reallocate funds for two FTEs and a vehicle to perform the duties internally. BAR #23-030 in the amount of $22,000 will appropriate the budget for the purchase of the vehicle.  In addition, $150,000 will be transferred to the operating account line to cover the cost of the two new grant-funded FTE’s.

This initiative will seek to mitigate the effects of chronic disease, increase access to healthy activities, reduce food insecurity, address social determinants of health, and provide for all at-risk populations regardless of socio-economic status.

The Community Health Initiative - Health Lifestyles program will empower our residents to make informed, healthy lifestyle choices through the promotion of health and wellness activities available via existing community-based programs and expand access to these programs to all underserved segments of the community.

Elements of the Healthy Lifestyles program:

                     Improving the provision of healthy lifestyle information and educational materials to individuals, families, and community groups to foster improved community health.

                     Building a shared understanding that lack of access to educational programs, public recreational facilities, and other resources can lead to uninformed risky behavior.

                     Focusing on the most critical priority areas that perpetuate unhealthy lifestyles such as poor nutrition, sedentary lifestyles, and use of addictive substances.

The CHCMs will work primarily in the field, in client homes, at outreach events, and collaborating with community providers. Both positions will be classified as grant-funded FTE and will be eliminated upon completion of the ARPA Community Health Program.
