Background:
RFP-5187-26 will provide medical screening, evaluation, and management of medical services that include annual physicals, pre-employment physicals, and return to work physicals.
The project was publicly advertised, and the County received three (3) responses listed alphabetically below.
• Adventist Health System/Sunbelt, Inc. dba AdventHealth Centra Care
• Bio-Care, Inc.
• Life Extension Clinics, Inc. D/B/A Life Scan Wellness Centers
The Evaluation Committee consisting of Christina Brandolini, Director of Human Resources; Matthew Hettler, Deputy Chief; and Ben DeCuir, Deputy Chief evaluated the responses giving consideration to qualifications, education, certifications, experience, understanding & approach, and pricing proposal.
The Evaluation Committee recommends award for this project to Adventist Health Systems/Sunbelt, Inc dba AdventHealth Centra Care and Life Extension Clinics, Inc dba Life Scan Wellness Centers, the top-ranked firms.
Authorization for performance under this Agreement will be in the form of written Purchase Orders issued by the County. The Agreement will take effect on the date of its execution and remain in effect for three (3) years. At the sole option of the County, the Agreement may be renewed for two (2) successive periods not to exceed one (1) year each. The estimated annual amount for this project is $533,000.

Requested Action:
Staff requests that the Board award RFP-5187-26/RLH Fire Department Physicians Services to Adventist Health Systems/Sunbelt, Inc dba AdventHealth Centra Care and Life Extension Clinics, Inc dba Life Scan Wellness Centers at an estimated annual amount of $533,000 and authorize the Purchasing and Contracts Division to execute the Agreement.
